It is good practice to provide the video in at least 2 different formats in case the visitor's browser does not support one. The browser will select the first format that it supports.

I recommend putting the .mp4 last as many browsers do not fully support mp4 videos. Video ogg is a good choice as it is fully supported by more browsers than mp4.
